침상 심장 초음파를 이용한 중환자실 환자의 혈역학적 평가

Trans Abstract

Bedside echocardiography has become a useful tool for hemodynamic assessment of patients in the intensive care unit (ICU). Direct visualization of the cardiac anatomy and functions as well as non-invasive measurement of doppler hemodynamics helps not only to diagnose cardiac problems (such as global or regional left ventricular function, valvular heart disease, and pericardial effusion), but also to determine hemodynamic status (such as volume status). The easy accessibility, non-invasiveness, and portability allow periodic monitoring during critical ICU periods. Therefore, increasing requests from ICU physicians make bedside echocardiography a point-of-care especially in hypotensive patients. This review will discuss echocardiography-based assessment of ICU patients, focusing on patients with hypotension or mechanical ventilators.

서 론

심장 초음파는 비침습적으로 심장의 구조적, 기능적 이상을 가하고, 각종 내과적 질환에 동반될 수 있는 혈역학적 변화를 평가하므로, 내과 의사에 있어서 청진기와 같은 필수적인 진단 도구가 되었다. 특히 혈역학적으로 불안정한 환자들을 치료하는 내과계 중환자실에서는 point-of-care ultrasound라는 개념이 보급되었고[1], 실제로 중환자실 담당 의사 선생님들로부터 침상 심장 초음파를 스스로 수행하고 싶다는 요구를 적지 않게 듣고 있다. 중환자실 환자들에서의 침상 심장 초음파의 요구는 우연히 심장질환을 발견하려는 것보다는 목표 지향적이었고, 주로 혈압이 불안정한 상황에서, 침상에서 바로 일차적이고 응급한 심장 원인을 배제하고, 체내 수분 상태를 파악하며, 심장 수축력 증강 약제를 쓸 것인지, 정맥 수액을 급속히 줄 것인지, 말초혈관 수축제를 처방할 것인지, 또 언제 수액을 줄여야 하는 것인지 등 변화하는 상황에 대처하여 치료적인 결정에 도움을 받고자 하는 경우가 대부분이었다.

침상 심장 초음파는 혈역학적으로 불안정하거나, 특히 각종장비와 인공호흡 기계 등의 도움을 받고 있는 이동이 어려운 중환자실 환자에서 쉽게 적용할 수 있는 검사법으로서[2], 본 종설은 이러한 중환자실 환자에서 동반되는 실제 임상적 상황에서 침상 심장 초음파의 적용을 보이고자 한다.

혈압이 저하된 경우의 침상 심장 초음파의 적용

좌심실 수축 기능의 평가(left ventricular [LV] systolic function): ejection fraction (EF)

좌심실 수축 기능의 지표 중에서 가장 널리 쓰이고 있는 것은 좌심실의 내경 및 용적에서 산출된 좌심실 구혈률(LV EF)이다. 대개 M-mode 또는 2D 영상을 이용하는데, 이완기말 좌심실이 가장 커져 있을 때와, 수축기말 좌심실이 가장 작아졌을 때의 내경 또는 용적을 측정하여, 각각 해당하는 수식에 의해 산출된다(M-mode, Teicholz formula: 2D, Biplane Modified Simpson’s Method) (Fig. 1) [3]. 공식은 기억하지 않아도 좋으며, 다만 Fig. 1과 같이 M-mode에서 거리, 2D에서 내벽의 tracing만 하면 기계가 산출해주는 알고리듬을 잘 익혀두기를 바란다. 또한, 침상에서 불안정한 검사환경과 밝은 조명 등으로 정확한 심근 내벽의 tracing이 어렵다면, 오히려 움직이고 있는 좌심실을 정성적으로 눈으로 보고 좌심실 수축기능을 평가하는 것이 더 유용할 수 있다. 정상 LV EF는 남자 52-72%, 여자 54-74%이며, EF 30% 미만은 중증 수축력 저하로 볼 수 있다[3].

육안으로 어느 심근벽이 상대적으로 수축이 잘 안 되는지 국소 벽운동 장애(regional wall motion abnormality)를 찾아내는 것도 도움이 되며, 이러한 경우는 해당되는 영역의 관상동맥 관류에 문제가 있는 허혈성 심질환을 먼저 의심하게 한다. 특히, 응급 관상동맥 조영술이 요구되는 급성 심근경색의 경우에는, 침상 초음파상 국소벽 운동장애와 동반하여 시시각각 변화하는 심전도가 특징적이므로, 중환자실 침상 심전도를 주기적으로 함께 모니터링하여야 한다. 다른 예로, 좌심실 심첨부에 국한된 벽운동 장애(apical ballooning)는 스트레스성 심근증(stress cardiomyopathy)을 의심하게 하는데, 한국과 일본은 서양에 비해 발생률이 높아서 감별 진단으로 고려하여야 한다[4,5] (Fig. 2). 한국의 삼차 의뢰기관 내과계 중환자실에서 각각 2 8% [4], 19% [5] 빈도로 보고된 바 있으며, 원인으로서는 폐렴, 패혈증이 빈번하였다.

일차적으로 EF가 좋고 국소벽 운동장애가 없으면 심장의 일차적인 원인이 아닐 것이라 생각할 수 있지만, 드물게 급성 승모판막 역류와 같은 환자에서는 super LV EF를 가진 급성 폐부종으로 나타날 수 있으므로, 순서를 가지고 2D 및 doppler를 통해서 전반적으로 심장의 기본적인 구조와 기능을 평가하는 안목이 필요하겠다.

좌심실 이완 기능 평가(LV diastolic function): mitral E/A, septal e’, E/e’

심장 주기에서 이완기는 심실 근육이 이완하고 심방에서 심실로 혈액이 유입되는 시기로서, 좌심실이 채워질 때, 압력이 과도하게 올라가면 폐부종이 발생할 수 있는데, 좌심실 충만압(LV filling pressure)이 높아져 있는 것을 평가하는 것이 이완기 심부전 진단에 있어 핵심적인 요소이다. 중환자실 침상 심장 초음파의 제한적 환경에서 빨리 이완기 장애를 파악하는 팁으로서, 만성적인 이완기 장애는 좌심방의 크기가 4 cm 이상 커져 있는 것으로, 현재 급성으로 좌심실 충만압이 올라가 있는 정도는 초기승모판 유입 혈류(mitral inflow E-wave velocity) 및 승모판륜 운동(mitral septal annular tissue doppler e' velocity)을 측정하여, E/e’ ratio > 15로 평가하는 것을 권한다(Fig. 3). 최근 미국 심장초음파 가이드라인에서는 좌심방 용적 지수(left atrial volume index, > 34 mL/m2)와 승모판륜 양쪽 e’ velocity의 평균치를 적용(septal a nd lateral a nnulus, averaged E/e’ r atio > 14)하고 있지만[6], 다수의 변수 측정 및 supine p osition상 lateral annular doppler angle 제한성 등을 고려한다면, 먼저 제시한 권고가 침상 초음파에서는 보다 임상적 유용도가 높을 것이라 판단된다. 대략 E/e’ = 15이면, LV filling pressure = 20 mmHg에 해당한다(mean pulmonary capillary wedge pressure = 1.91 + [1.24 × E/e’])[7]. 특히 E/e’ ratio는 치료에 따른 좌심실 충만압의 변화 추적에도 도움을 주는데, 폐부종을 동반한 이완기 장애 2도 환자에서 이뇨제 치료 이후 다음날 이완기 1도로 변하는 예를 종종 볼 수 있다. 이완기 2도 이상에서는 정주 수액에 의해 폐부종이 초래될 수 있음을 유의하여야 한다. 고령이나 고혈압 환자에서 이완기 1도는 일반적으로 볼 수 있고, 그 자체로 치료의 대상은 아니다.

만약, 침상 심장 초음파 기계가 조직도플러 e’ velocity를 측정할 수 있는 모듈이 없다면, 심장의 크기와 수축 기능 그리고 mitral inflow 패턴으로 좌심실 이완 기능을 미루어 짐작할 수 있다. 비록 mitral inflow E/A > 1이 정상처럼 보인다고 할지라도, 동반된 소견 즉 노인에서 좌심방이 크거나, 좌심실이 크고 수축기능이 저하되어 있다면 좌심실 충만압이 올라가 있음을 정성적으로 추정할 수 있다.

좌심실 유출로 압력 차 평가(LV outflow obstruction)

혈압이 저하된 중환자에서 inotropic support와 fluid therapy를 놓고, 수액 과다에 의한 폐부종 등이 염려되어, inotropic agents를 더 선호하는 경향이 있다. 즉 혈압 저하 환자에서 도파민과 같은 일종의 승압제를 원인에 관계없이 정주하게 되는 경우가 있는데, 도파민 정주를 하면 할수록 혈압이 더 저하되는 경우가 있다. 이는 혈압 저하 중환자에서 침상 심장 초음파검사가 치료의 가이드가 되는 좋은 예가 될 수 있는데, 좌심실 유출로의 압력 차가 발생되는 경우, 도파민 정주로 인해 압력 차가 더 악화되어 전신 혈압 저하, 쇼크에 빠질 수 있음은 예측 가능하고 방지할 수 있다[8,9]. 이때 침상 심장 초음파는 승모판 전엽의 수축기 전방 이동과 더불어 발생되는 좌심실 유출로 압력 차를 평가하고[10] (Fig. 4), dynamic obstruction 존재에 따라, 혈관 내 볼륨을 정주하고, 오히려 도파민과 같은 약제를 끊게 되는 판단의 근거를 제공하므로, 치료적 측면에서 중요한 평가 도구라 할 수 있다.

우심실 확장 및 폐고혈압 정도의 평가 (pulmonary hypertension)

우심실의 크기와 기능, 압력에 대한 평가는 이를 초래하게 된 원인적인 요소(폐동맥혈전색전증, 폐질환, 폐울혈, 좌심실 충만압 상승) 파악에 도움을 준다. 침상 심장 초음파에서 우심실의 평가는 대부분 visual estimation으로 시작하게 되는데, 만약 우심실이 좌심실의 크기와 같다면, 이미 우심실은 중등도 확장되어 있음을 의미한다. 내과계 중환자실에서 폐동맥 압력 측정을위해 침습적인 스완간즈 카테러를 삽입하는 것은 더 이상 보기 어렵다. 폐동맥 수축기압은 우심실 유출로 폐쇄가 없다면 우심실 수축기 압력과 같고, 비침습적으로 도플러 심초음파 삼첨판 역류 속도로 산출할 수 있다(Fig. 5). 연속파 도플러로 삼첨판 역류제트의 최고속도를 재면, modified Bernoulli equation에 의하여 우심방과 우심실 압력 차를 구할 수 있고(pressure gradient= 4V2), 여기에 우심방 추정압력을 더하여 폐동맥 수축기압을 산출한다[11,12]. 간단히 말하면, 삼첨판 역류 속도 Vmax ≤2.8 m/s이면 폐고혈압 존재의 가능성이 거의 없으며, Vmax >3.4 m/s이면 중등도 이상의 폐고혈압을 시사한다. 이는 원인에 대한 고찰 및 치료 경과 관찰에 유용하다. 주말 동안 고혈당 대사성 산증으로 응급실을 통해 입원한 내분비내과 환자에서, 초음파검사를 통해 우심실이 좌심실보다 크고, 우심실 수축기압이 52 mmHg로 중등도 상승되어 있어, 폐동맥혈전을 의심하여 조영 CT를 시행하였고, 급성 폐혈전색전증으로 확진하였으며 헤파린 정주치료 이후 시행한 심장 초음파에서 우심실 수축기압이 저하됨을 관찰하였던 예를 들 수 있겠다.

혈관내 수분 상태의 평가(intravascular volume status)

중환자실 환자에서 혈관내 수분 상태를 파악하여 적절한 volume loading을 하는 것은 치료에 필수적이다. 그래서 대개 중환자실에서는 침상 중심 정맥압(central venous pressure)을 모니터링하는데, 이에 침상 심장 초음파를 적용하면, 하대정맥(inferior vena cava, IVC), 우심장 크기 등을 통해 preload status를 간접적으로 평가할 뿐 아니라, 좌심실 수축력 및 이완기 압력, 폐동맥압에 연결되는 혈역학을 이해할 수 있고, 호흡에 따른 IVC collapse 또는 fluid bolus로 주었을 때 심박출량 변화도 평가할 수 있어 동적인 수분 반응 평가에도 도움이 된다[13,14]. 적절한 수분 상태에서는 자발 호흡의 흡기 시에 하대정맥 직경이 50% 이상 감소됨을 볼 수 있다. 그러나 혈관내 수분 상태가 낮았을 경우에는, 초음파상 우심실 용적이 적으며, 하대정맥이 호흡에 관계없이 내내 붙어 있음을 관찰할 수 있다(Fig. 4C and 4D).

심장 눌림증(cardiac tamponade), 심낭 삼출(pericardial effusion), 흉막 삼출(pleural effusion)

심장 눌림증은 심낭 삼출액으로 인한 심낭압의 증가가 심장의 filling을 저해하고 결국 임상적으로는 전신 혈압 저하로 나타나는 응급 상황이다[15]. 혈압 저하 환자에서 침상 심장 초음파로 심낭 삼출액을 발견하였을 때, 삼출액의 양과 혈역학적 영향을 가늠해야 한다(Fig. 6). 심낭액의 양은 2D에서 echo-free space를 측정하여 반정량적으로 기술한다: 극소(trivial, 수축기 때만 보임), 소량 (small, < 10 mm), 중등도(moderate, 10-20 mm), 과량(large, > 20 mm) [16]. 심낭액의 혈역학적 평가로 tamponade를 시사하는 소견으로는, 우심실의 이완기 전반 collapse, 우심방의 이완기 후반 1/3 이상에서 invagination, 하대정맥 확장 및 흡기시 무변화, 승모판 E velocity 흡기시 저하 > 25% 등이 있다[16]. 침상 심장 초음파는 심장 눌림증 진단 즉시 필요한 심낭 천자가 잘 수행되도록 천자 바늘을 가이드하며, 시술 과정 및 결과를 실시간 모니터링 가능하게 한다[17].

그 외, 심장 주위에 동반될 수 있는 액체로 흉막 삼출액도 있는데, 저산소성 호흡부전 환자에서 원인의 하나로 의심할 수 있다. 심낭 삼출액과 흉막 삼출액을 구분하는 구조물의 하나로, 하행 대동맥이 언급되기도 하지만(Fig. 7A), 침상 심장 초음파에서는 직접 프로브를 들고 우측, 좌측 폐야에 탐촉자를 대어보는 것을 권고한다. 누워 있는 상태에서 아래로 깔려 있는 흉수 및 들숨에 의해 폐가 정상적으로 확장되지 않는 소견까지 더불어 관찰할 수 있다(Fig. 7B and 7C).

인공호흡 기계 치료를 받고 있는 환자의 특성

늑골 하방 검사(subcostal view)

중환자실 침상 심장 초음파검사를 시행함에 있어서 ‘poor acoustic window’는 극복하여야 할 장애물 중의 하나이다. 환자가 침상에 똑바로 누운 자세에서 인공호흡기 치료를 받고 있는 경우, 흔히 과팽창된 폐가 심장 앞쪽을 커버할 수 있다. 이러한 경우, 폐(공기)는 인체에서 감쇠 계수가 가장 큰 조직으로서(attenuation coefficient: lung 40.0 vs. water 0.002 dB/cm MHz), 폐 뒤에 놓인 심장까지 초음파 빔이 닿지 않아서 적절한 심장영상을 얻을 수 없다[18]. 또한, 과팽창된 폐로 인하여, 마치 만성 폐쇄성 폐질환 환자에서와 같이, 심장은 아래로 밀리게 되는데, 이때는 늑골하방 subcostal view를 이용하면 가장 좋은 심장 영상을 만들 수 있다(Fig. 8). 중환자실 침상 심장 초음파를 시행하면서 subcostal view가 얻을 수 있는 유일한 window인 경우도 있으며, 다행히 subcostal view에서 기본적인 parasternal, apical views와 유사한 2D 단층 촬영면을 구현할 수 있고, 심장의 크기, 기능, 국소벽 운동이상, 판막, 폐동맥까지도 평가할 수 있음은 주지할 사실이다(Fig. 8).

호흡시 동적인 변화(respiratory variation)

자발 호흡 환자와 비교할 때, 인공 양압 호흡 환자에서는 호흡시 동적인 변화 평가에 있어 주의를 요한다. 하대정맥 호흡반응의 경우, 자발 호흡 시에는 흡기시 흉강내압이 저하되면서 하대정맥에서 우심방으로 혈액이 유입되면서 하대정맥이 collapse되는 것을 기대하는데, 양압 호흡에서는 흡기시 흉강내압이 상승되면서 혈액이 심장에서 하대정맥으로 쏠려서 오히려 하대정맥이 확장될 가능성이 있다. 유사한 예로서, 심장 눌림증 환자에서 판막을 통한 혈류의 호흡성 변화의 적용도 들 수 있다. 그럼에도 불구하고, 기계호흡 환자에서 하대정맥의 호흡 변화 12-18%는 양호한 수액반응(즉, fluid bolus에 의해 심박출량 > 10% 증가)을 반영한다고 보고되고 있다[13,19].

인공호흡기 이탈 과정(weaning process)

인공호흡기 치료를 받는 환자에서 원인되는 병적 상태가 해결되어 기계호흡기를 없애는 과정을 이탈 과정(weaning process)이라 한다. 인공호흡기 이탈 실패 환자에서, 양압 호흡에서 자발호흡으로 이행하면서 겪어야 하는 심장의 부담이 이유가 될 수 있는데, 이때 침상 심장 초음파검사가 적용될 수 있겠다. 일반적으로, 침상 심장 초음파는 weaning에 필요한 negative water balance 여부와 좌심실 충만압이 증가되지 않도록, 폐동맥 압력이 증가되지 않도록, 즉 폐부종이 발생되지 않도록 이탈 단계마다 조절하는 과정에서 지속적으로 혈역학적 상태를 평가하여 도움을 준다[20].

결 론

중환자실에서 침상 심장 초음파는 비침습적으로 심장질환에 대한 진단뿐만 아니라 실시간 볼륨 상태에 대한 정보를 준다. 특히 기계호흡 환자에서는 늑골하방 검사가 도움이 된다. 도플러 적용을 통한 좌심실 압력 및 폐고혈압 정도 파악 및 하대정맥 영상은 정적 및 동적인 혈역학적 평가에 유용하다. 임상 의사의 머리와 손으로 만들어지는, 즉 목적에 맞는 임상적인 해석과 치료 방향 결정에 도움을 주는 침상 심장 초음파 검사법은중환자실 환자의 돌봄에 있어서 필수 불가결한 도구라 할 수 있겠다.
