척추관절염의 부착부염에 대한 근골격 초음파의 활용

Abstract

Spondyloarthritis (SpA) consists of ankylosing spondylitis, psoriatic arthritis, reactive arthritis, enteropathy-related spondylitis and arthritis, and undifferentiated SpA. It is accompanied by inflammatory back pain and joint symptoms such as oligoarthritis, dactilitis, and enthesitis, which usually occur in the lower extremities. Among these symptoms, enthesitis is an important clinical symptom of SpA because early diagnosis and treatment of enthesitis may improve the prognosis. Therefore, it is important to evaluate objectively the treatment response of enthesitis. Recently, semi-quantitative ultrasonographic evaluation has been used for this purpose by using B-mode images and power Doppler techniques. In this article, I describe the role and application of ultrasonography in the enthesitis of SpA.

서 론

척추관절염(spondyloarthritis)은 강직척추염, 건선관절염, 반응관절염, 염증장질환 연관 관절염 및 미분화 척추관절염으로 구성된 질환군으로 HLA-B27 유전자와 연관성이 있으며 염증성 허리 통증, 주로 하지에 동반되는 소수관절염(oligoarthritis), 가락염(dactilitis) 및 부착부염(enthesitis)과 같은 관절 증상 및 포도막염, 염증장질환 또는 건선과 같은 관절 외 증상을 포함하는 다양한 임상증상을 동반한다[1]. 이러한 임상증상 가운데 부착부염은 척추관절염의 중요한 특징 가운데 하나이며 부착부(enthesis)로부터 관절의 활막과 인대까지 염증이 전파될 수 있다[2,3]. 또한 부착부염은 척추관절염에 있어서 활성도와 치료에 대한 평가를 반영하게 된다[4,5]. 척추관절염 환자에서 동반되는 부착부염을 평가하는 데 있어 이학적인 검사만으로는 염증에 대한 객관적인 평가를 시행하는 것에 제한이 있어 이를 보안하기 위해 자기공명영상 혹은 초음파검사와 같은 영상검사를 도입하게 되었다[5]. 초음파는 방사선을 발생하지 않고 비용적인 장점이 있으며, 재현성이 우수하고 환자와 의사에서 수용력이 좋은 기술적인 장점이 있다[6]. 또한 다양한 부위를 실시간으로 검사를 진행하고 영상을 제공할 수 있어 활막염과 부착부염을 확인하는 데 있어 임상검진에 비해 민감도가 우수한 장점이 있다[6]. 또한 초음파를 활용한 관절액 천자와 주사를 시행할 수 있으며 그 효과를 증가시킬 수 있다[6]. 척추관절염의 부착부에 대한 초음파검사는 1994년 Lehtinen 등에 의해 처음 시도되었으며[6], 이후 2002년에 D’Agostino 등[5]이 파워도플러검사를 부착부염을 평가하는 데 도입하여 뼈의 부착 부위에 비정상적인 혈류를 확인하는 것으로 부착부의 염증 반응의 활성도를 평가하게 되었다. 이후 초음파기기의 발달과 함께 Outcome Measures in Rheumatoid Arthritis Clinical Trial (OMERACT)에서 부착부병증(enthesopathy)에 대한 정의를 발표하는 것이 척추관절염에서 부착부염에 대한 초음파 소견을 기술하는 데 중요한 역할을 하게 되었다[6]. 따라서 본 논문에서는 척추관절염에서 공통적으로 동반될 수 있는 부착부염에 대한 초음파검사에 대한 전반적인 내용과 함께 치료에 대한 평가 방법에 대하여 알아보고자 한다.

본 론

부착부와 부착부염에 대한 정의

부착부는 관절의 움직임에 중요한 역할을 하며 인대(tendon), 건(ligament), 관절강( joint capsule), 근막(fascia) 혹은 근육(muscle)이 뼈에 붙는 부위를 말한다[2,5,6]. 이 부위는 전이조직(transition tissue)으로 연부조직이 부착하는 것 이외에도 부착부위에 전달되는 스트레스를 주위의 뼈 등에 전달하거나 그 반대의 역할을 하게 된다[6]. 부착부는 두 가지, 즉 섬유(fibrous) 혹은 섬유연골(fibrocartilaginous)로 구성되어 있다. 섬유 부착부는 주로 긴 뼈의 골간단(metaphysis) 혹은 골간(diaphysis)에 부착하며 인대와 뼈 혹은 골막(periosteum)사이에 치밀한 섬유결합조직으로 구성되어 있다[7,8]. 섬유연골 부착부는 전형적으로 골단(epiphyses), 견인골단(apophysis) 혹은 작은 뼈에 부착되며 석회화된 성분을 가지고 있는 경우도 있다[7]. 척추관절염에서 동반되는 부착부염은 주로 섬유연골 부착부에서 동반되는 것으로 알려져 있다[6]. 부착부병증(enthesopathy)은 부착부에 다양한 원인, 예를 들어 기계적 외상, 대사성 혹은 염증 반응 등에 의한 구조적인 변화를 통칭하는 경우에 사용되는 개념이며 반면에 부착부염은 염증 반응에 의한 변화가 다른 원인에 의해서도 발생할 수 있지만 척추관절염과 같은 질환에 의해 활성을 보이는 염증 반응이 부착부에 동반된 경우에 사용된다[6]. 부착부병증에 대한 초음파 소견은 2005년 유럽연합(European League against Rheumatism)과 OMERACT에서 골미란, 관절의 부종, 윤활막비대, 윤활막염에 대한 명명법과 함께 검사 방법에 대한 초음파적인 정의에 대한 합의를 제시하였다[9] (Table 1).

부착부염의 초음파검사

부착부염을 확인하기 위해 사용되는 영상검사는 단순방사선 사진, 컴퓨터단층촬영, 자기공명영상 및 초음파 등이 활용되고 있다. 단순방사선사진 혹은 컴퓨터단층촬영을 이용한 부착부염에 대한 평가는 단순히 뼈의 구조적인 변화만을 확인하게 되는 제한점이 있다[6]. 그리고 자기공명영상은 주로 척추와 천장골염의 염증을 평가하는 데 사용되고 있으며, 초음파는 부착부의 염증을 포함한 다양한 평가를 하기 위해 활용되고 있다[6]. 부착부염을 초음파로 평가하기 위해서는 부착부를 구성하는 인대와 건과 같은 내부 구조물을 초음파로 정확한 영상을 획득하기 위한 준비가 필요하다. 표면에 위치한 인대를 관찰하기 위해서는 초음파의 주파수와 이에 따른 침투력이 중요하며 일반적으로는 10-12 MHz의 선형탐촉자를 추천하며 부착부염을 진단하고 평가하는 데 있어 최근에는 gray scale 이외에 파워도플러(Power Doppler)를 이용하여 인대에 염증의 정도를 평가하고 있다[7]. 그리고 이러한 초음파검사는 다수의 문헌에서 부착부염을 진단하는 데 있어 특이도와 민감도가 우수하다고 알려져 있어 척추관절염 환자의 부착부염을 객관적으로 평가하기 위해 최근에 활용도가 증가되고 있다[10-12]. 회색 모드(gray scale mode)에서 관찰되는 부착부염의 소견은 윤활낭염(bursitis), 부착부골극(enthesophytes), 정상적인 근섬유 에코결(fibrillar echotexture) 및 에코발생(echogenesity) 소실, 피하조직의 부종, 부착부의 비후, 골 미란[12,13] 등의 소견이 관찰된다(Fig. 1).
처음으로 부착부에 대한 광범위한 초음파 소견을 기술한 논문은 1994년에 Lehtinen 등[14]이 발표하였다. 이 문헌에서는 31명의 척추관절염 환자를 대상으로 초음파검사를 시행하였으며 가장 흔하게 침범하는 부위는 아킬레스인대와 족저근막이었다[14]. 이후 2002년에 Balint 등[15]에 의해서 35명을 대상으로 슬개골의 위쪽과 아래쪽 끝, 경골조면, 아킬레스인대와 발바닥널힘줄을 초음파로 평가하였으며 이학적 검사와 비교하여 초음파검사가 부착부염을 탐지하는 데 더 우수하였다고 발표하였다. 이후 2008년에 Scarpa 등[16]은 건선관절염 환자를 대상으로 초음파 연구를 시행하였으며, 이 연구에서도 기존의 연구와 유사한 결과를 보고하였다. D’Agostino 등[17]은 2003년에 부착부염을 평가하기 위하여 대조군으로 기계적 요통 환자와 류마티스관절염 환자를 포함하고 회색 모드와 파워도플러를 함께 시행하고 결과를 비교하였다. 이 논문에서는 초음파검사가 척추관절염의 진단과 활성도를 평가하는 데 있어 유용하게 활용될 수 있는 검사로 평가하였다. 이후 D’Agostino 등[18]은 2011년에 전향적인 연구를 통해 적어도 한 군데 이상에서 파워도플러 신호가 나오는 부착부염은 척추관절염을 진단하는 데 있어서 중요한 예측인자가 됨을 발표하였다. 2011년 Spadaro 등[13]에 의해 36명의 강직척추염 환자를 대상으로 432군데의 부착부를 확인하였으며, 이학적 검사(14,8%)에 비해서 파워도플러(44,4%)가 부착부의 이상 소견을 확인하는 데 이학적 검사보다 우수하다는 결과를 보고하였다. 그리고 2015년에 유럽연합에서는 척추관절염을 진단하고 치료하는 데 있어서 영상검사의 활용에 대한 권고안을 제시하였고, 말초 척추관절염(peripheral SpA)의 경우 활막염과 부착부염에서 초음파와 자기공명영상이 질환의 활성도를 반영하는 데 활용될 수 있다고 명시하였다[19]. 그리고 최근에 Fujikawa 등[2]이 2019년에 발표한 자료에 의하면 136명의 척추관절염 환자를 대상으로 부착부염을 파워도플러를 이용하여 초음파검사를 시행하였으며, 척추관절염의 말초관절염 증상을 동반한 환자에서 초음파검사가 진단에 유용하다고 보고하였다. 이러한 다수의 문헌을 바탕으로 척추관절염 환자에서 회색 모드 혹은 파워도플러를 함께 활용한 초음파검사는 부착부염에 대한 진단 및 치료에 대한 반응과 같은 부착부에 동반된 염증을 평가하는 데 있어 실제 임상에서 유용하게 활용될 수 있을 것으로 판단된다.
부착부염은 척추관절염 가운데 하나인 건선관절염에서도 초기에 동반될 수 있는 임상증상 가운데 하나로 알려져 있으며 건선관절염 역시 조기진단이 장기적인 예후와 연관이 있다[20]. 이러한 이유로 건선관절염 환자를 대상으로 한 초음파 연구도 다수 발표되고 있다[21-23]. Freeston 등[23]이 발표한 자료에 의하면 건선관절염 환자에서 부착부에 대한 이학적 검사와 초음파검사와는 일치도가 낮았으며 오히려 이학적 검사가 활성도를 과하게 나타낼 수 있다고 기술하였다. 또한 2017년에 Michelsen 등[21]은 141명의 건선관절염 환자의 아킬레스 인대를 평가하였으며 이학적 검사에서 압통이 88군데(31.2%)에서 확인이 되었지만 초음파검사에서 염증의 활성을 보인 것은 46군데(16.3%)로 이 문헌에서도 이학적 검사와 초음파검사 간의 일치도가 낮음을 발표하였다. 따라서 건선관절염 환자의 부착부를 단순히 이학적 검사만을 시행하는 것보다는 초음파검사와 함께 평가를 시행하는 것이 부착부염에 대한 정확한 평가를 하는 데 도움이 될 것으로 생각된다.

부착부염 평가 방법 및 활용

다수의 반정량적 검사 방법이 부착부의 초음파 이상을 평가하는 데 사용되고 있으며 이 가운데 4개의 평가 방법을 주로 활용하고 있다. 주로 활용되고 있는 방법은 Glasgow Ultrasound Enthesitis Scoring System (GUESS), D’Agostino scoring system, Spanish Enthesitis Index (SEI), Madrid Sonographic Enthesitis Index (MASEI)이다[24]. 이 가운데 가장 흔하게 사용되는 평가방법은 GUESS 방법으로 하지의 5군데의 부착부를 양쪽으로 평가하였다. 평가하는 부위는 대퇴사두근 부착부, 근위무릎인대 부착부, 원위무릎인대 부착부, 아킬레스인대 부착부, 족저근막 부착부(plantar aponeurosis)이며 인대의 두께, 윤활낭염, 골미란, 부착부골극에 대해 초음파의 회색 모드만으로 평가하였다[15]. 또 다른 방법은 D’Agostino scoring system으로 이는 회색모드와 도플러를 함께 적용하여 도플러 신호의 정도와 구조적인 손상의 유무에 따라 중증도를 평가하였다[17,25]. SEI는 GUESS 방식을 확장한 방법으로 에코발생, 건의 파열, 두께의 감소와 부종을 회색 모드로 평가를 하였고, 활성을 보이는 염증 반응(SEI-A)과 만성적인 반응(SEI-C)으로 분류하였다[15]. 그리고 M ASEI는 유일하게 부 착부병증을 OMERACT의 정의에 따라 기술하고 있으며 회색 모드와 파워도플러를 적용하여 평가하고 있고 점수가 18점 이상인 환자에서 척추관절염을 진단하는 데 있어서 민감도는 83.3%, 특이도는 82.8%로 높게 보고하였다[6].
초음파는 강직척추염의 치료 이후 부착부염의 변화를 관찰하는 데 높은 민감도를 제시하였다[24]. 특히 도플러를 추가하게 되면 더 우수한 반응을 보였으며 이러한 이유로 도플러는 치료에 대한 반응을 평가하거나 부착부를 평가하는 데 있어 중요하게 활용되고 있다[26-28]. Aydin 등[26]이 발표한 문헌에서는 43명의 강직성 척추염 환자를 대상으로 회색 모드와 파워도플러를 이용하여 anti-tumor necrosis factor (anti-TNF) 제제로 2개월간 치료 이후에 치료에 대한 반응을 평가하였으며, 초음파 검사에서 부착부염이 호전을 보이는 것을 초음파를 활용하여 확인하였다. Naredo 등[27]은 327명의 척추관절염 환자를 대상으로 6개월간 anti-TNF 제제를 투약한 이후 치료에 대한 반응을 회색모드 혹은 파워도플러검사로 비교하였고 파워도플러 신호가 치료에 대한 효과를 반영한다고 발표하였다. 또한 Huang 등[28]이 발표한 문헌에서 anti-TNF 제제로 치료 혹은 초음파유도로 주사치료를 시행한 이후에 치료에 대한 반응을 평가였으며 치료 이후에 혈류의 신호가 감소되는 것을 확인할 수 있었다. 따라서 도플러검사를 이용한 초음파는 척추관절염의 치료에 대한 반응을 관찰하는 데 유용하게 활용될 수 있을 것으로 생각된다.

결 론

부착부염은 척추관절염의 중요한 증상 가운데 하나이며 관절의 손상과 같은 장기적인 예후와 연관이 있다. 이러한 이유로 부착부염을 조기에 정확하게 진단하고 또한 치료에 대한 반응을 평가하는 것이 환자의 조기 치료와 예후에 중요하게 작용하게 되었다. 이러한 이유로 부착부염을 객관적으로 평가하기 위하여 다양한 영상검사가 활용되고 있다. 이 가운데 실제 임상에서 활용도가 높은 초음파가 중요한 검사로 자리잡고 있으며 회색모드 이외에도 파워도플러를 활용하여 염증 정도를 평가하고 있다. 따라서 부착부염에 대한 근골격 초음파의 활용도는 실제 임상 특히 외래진료에서 유용하게 활용될 수 있을 것으로 생각되며 척추관절염의 조기진단과 치료에 대한 반응을 평가하는데 있어 지속적으로 적용될 수 있을 것으로 생각한다.
