간종괴의 초음파 진단

Abstract

Ultrasonography (US) is a non-invasive, rapid, and simple examination to diagnose liver disease. The diagnosis of liver mass is often encountered on US examination and the differentiation between benign and malignant tumor is very important. We can perform the differential diagnosis of liver mass according to the sign of US. Hyperechoic rim around the edge of liver mass is shown in the hepatic hemangioma. Hypoechoic halo surrounding a tumor suggests probable malignancy, and multiple masses with target sign most often suggest hepatic metastases. A single multilocular cystic tumour with upstream bile duct dilatation or mural nodules is typically seen at the malignant cystic neoplasm. This review summarized the characteristics of US findings between benign and malignant liver mass.

서 론

복부 초음파는 비침습적 검사로서 간단하고 쉽게 복부검사를 시행할 수 있으며 조영제를 사용하지 않기 때문에 조영제 부작용이 있는 환자나 신기능이 저하된 환자에서도 유용하게 사용된다. 간질환에 있어서 복부 초음파검사는 검진을 위한 스크리닝 검사나 만성 B형간염 등을 포함한 만성 간질환 환자의 추적검사로 널리 이용되는데, 특히 간종괴는 이러한 초음파검사에서 종종 접하게 되는 중요한 소견이다. 간종괴는 종괴의 성상에 따라서 고형(solid) 종괴와 낭성(cystic) 종괴로 나눌 수 있으며, 특성에 따라 양성과 악성으로 분류할 수 있는데, 특히 양성과 악성의 감별진단은 복부 초음파검사에서 매우 중요하다. 양성 종양은 고형 종괴로는 혈관종(hemangioma), 국소결절성과형성(focal nodular hyperplasia), 간선종(hepatic adenoma) 등이 대표적이며, 낭성 종괴로는 단순성 간낭종(simple hepatic cyst), 간농양(liver abscess)과 간손상에 의해서 발생하는 간혈종(hepatic hematoma) 등이 있다. 악성 종양은 고형 종괴로는 간세포암종(hepatocellular carcinoma), 간내담관암(intrahepatic cholangiocarcinoma), 전이성 간암(metastatic liver cancer) 등이 대표적이며, 낭성 종괴로는 드물지만 혈관육종(angiosarcoma)과 담도낭선종/낭선암종(biliary cystadenoma/adenocarcinoma) 등이 있다. 본고에서는 간종괴에서의 복부 초음파 상의 특징을 초음파 사진과 함께 정리하였다.

본 론

양성 간종괴(benign liver mass)

양성 고형성 간종괴(benign solid liver mass)

혈관종(hemangioma)

전형적 혈관종(typical hemangioma)

전형적 혈관종은 초음파 상에서 고에코로 균일하고 주위와의 경계가 명확하다. 또한 후방 음향 증강(posterior acoustic enhancement)을 가지며, 종괴 주변부로 고에코의 변연(hyperechoic rim)을 갖는 것이 특징적이다(Fig. 1A, Table 1) [1,2]. 초음파의 고에코는 조직학적으로 혈관종을 구성하는 수많은 동(sinus)들 사이의 각각의 접촉면과 동(sinus)을 채우고 있는 혈액에 의해서 고에코를 띠게 된다. 이러한 고에코는 대개 작은 크기의 혈관종에서 볼 수 있다.

비전형적 혈관종(atypical hemangioma)

종괴의 크기가 커질수록 혈관종 내의 괴사나 출혈 또는 섬유화 등에 의해서 고에코와 저에코가 혼합된 양상의 불균일한 형태를 보일 수 있게 되고 그런 경우를 비전형적 혈관종(atypical hemangioma)이라고 한다(Fig. 1B-1D). 그러나 이러한 혼합에코의 종괴는 간선종이나 간세포암종 그리고 전이성 간암에서도 보일 수 있으므로 감별을 요한다. 초음파는 3 cm 이하의 악성 고에코 종괴와 혈관종을 감별하는데 있어서 민감도 94.1%, 특이도 80.0%의 좋은 정확성을 보인다. 도플러 초음파에서 대부분의 혈관종은 도플러 시그널이 없거나 미세한 시그널을 보여[3], 종괴 내부나 종괴 주위로 혈관성을 갖는 간세포암종과 차이를 보인다.

지방간에서의 혈관종

혈관종은 간의 에코가 증가된 지방간에서는 저에코로 관찰될 수 있다(Fig. 1E-1F). 이때는 국한성 비지방 침윤(focal fat sparing zone), 간세포암종, 전이성 간암, 간내담관암(intrahepatic cholangiocarcinoma) 등과의 감별을 요한다.

국소결절성과형성(focal nodular hyperplasia)

간에서 발견되는 드문 양성 종양으로 간선종(hepatic adenoma)과 마찬가지로 여자에서 흔하며 특히 가임기 여성에서 잘 발견된다. 무증상 환자에서 검진 시에 우연히 발견되는 경우가 많으며, 발달과정에서 동맥의 선천적 혈관기형의 과증식이 원인으로 생각되고 있다. 대부분의 병변은 5 cm 미만이며, 단일 종괴이다. 초음파 상에서는 약간의 고에코나 종종 정상 간조직과 구분이 어려운 등에코에 가까운 종괴를 보이며, 일부 종괴에서는 특징적인 소견으로 중심부 반흔(central scar)이 저에코의 별모양으로 관찰된다(Fig. 2A) [4]. Computed tomography (CT)에서는 동맥기에서 강하게 조영되었다가 문맥기 이후에서 조영제가 소실되는 소견이 관찰된다(Fig. 2B-2E). 초음파에서 중심부 반흔을 동반하지 않은 경우에는 국소결절성과형성의 진단 예측이 어려우며, 혈관종이나 간선종 그리고 다른 악성 종괴와의 감별이 필요하다.

간선종(hepatic adenoma)

드문 양성 간종괴로 악성으로 진행할 수 있는 가능성을 가지고 있다. 경구피임약과 관련이 있으며 여성에게서 흔하다. 초음파 소견은 간선종의 내부 출혈이나 국한성 비지방 침윤(focal fat sparing zone) 등에 의해서 고에코나 저에코로 나타날 수 있으며, 비특이적 소견으로 초음파상 감별이 어렵다(Fig. 3). 지방간에서는 등에코나 저에코로 관찰되며(Fig. 4A), CT에서는 동맥기에서 고르게 조영되며 문맥기와 지연기에서 간실질과 등에코를 보인다(Fig. 4B-4E).

양성 낭종성 간종괴(benign cystic liver mass)

단순성 간낭종(simple hepatic cyst)

단순성 간낭종은 최근 초음파검사 및 CT 등 건강검진의 증가로 우연히 발견되는 경우가 많은 양성 질환이다. 단순성 간낭종의 초음파상 특징은 낭종의 내부는 무에코이며, 낭종벽이 평활하고 특히 후벽의 윤곽이 명확하며, 낭종의 후부에코 증강을 갖는 것이 특징이다(Fig. 5).

간농양(liver abscess)

간농양의 초음파 소견은 간농양의 시기에 따라서 다양하게 나타날 수 있다. 일반적으로 간농양의 성숙이 진행될수록 충실성 에코양상에서 혼합성 에코양상을 보이다가 무에코의 낭종성 에코로 진행하게 되며 종괴의 경계도 시간이 지남에 따라서 뚜렷하게 구분이 된다. 아직 무에코로 진행되지 않은 상태의 충실성 에코양상을 보일 때는 간세포암종, 간내담관암, 전이성 간암 그리고 괴사성 간종괴 등과의 감별이 필요하다. 발열과 우상복부통증, 백혈구 증다증이 있으면 진단에 도움이 된다. 농양벽은 일반적으로 불규칙하고 지저분하며 농양 내부의 debris에 의한 에코를 보인다(Fig. 6).

간혈종(liver hematoma)

간혈종은 사고 등에 인한 간손상으로 발생한 출혈이 피막을 형성하여 둘러싸여진 상태로, 간의 어느 곳에서나 발생할 수 있지만, 주로 간우엽 전외측(anterolateral aspect of Rt. Liver)에 발생하며, 간의 피막과 실질 사이에 위치한다. 간혈종은 주위와의 경계가 분명한 렌즈폼의 형태를 이룬다. 복부 초음파 상에서 급성기에는 일반적으로 출혈에 의한 고에코 양상을 보이지만 만성기로 진행되면서 혈종 내부의 격벽과 debris에 의해서 불균일한 저에코 양상으로 나타난다(Fig. 7).

악성 간종괴(malignant liver mass)

악성 고형성 간종괴(malignant solid liver mass)

간세포암종(hepatocellular carcinoma)

소간암(< 2 cm)은 대부분이 조직학적으로 괴사를 아직 동반하지 않은 고형종(solid tumor) 상태이기 때문에 초음파에서 일반적으로 저에코로 관찰된다(Fig. 8A) [5]. 일반적으로 간암은 2 cm 미만에서는 테두리(halo)가 없는 저에코 종괴 소견을 보이다가, 2-3 cm의 크기로 커지면 종양을 싸고 있는 섬유화 캡슐(fibrous capsule)에 의해서 종양 변연부에 저에코의 얇은 테두리(thin, peripheral hypoechoic halo)를 보이게 된다(Fig. 8B) [6]. 종괴가 3-4 cm로 커지면, 종괴 주위의 테두리가 옅어지고 내부에 여러 개의 저에코를 동반하는 “nodules in tumor” 양상을 보인다(Fig. 8C). 종괴의 크기가 4-6 cm로 더 커지면, 내부에 있던 여러 개의 저에코의 종괴들도 점차 커지거나 모양이 바뀌면서 “tumors in tumor” 또는 모자이크 패턴의 모양을 갖게 된다(Fig. 8D). 일반적으로 종괴가 6 cm 이상으로 커지면, 점차 테두리가 소실되면서 대부분 경계가 불분명한 괴상형(massive type)이나 부정형의 종괴로 발전하게 된다(Fig. 8E, Table 2). 시간이 지나고 종괴가 커질수록 좀 더 복잡하고 불균일한 성상을 보이게 되는데, 이는 괴사와 섬유화가 지속적으로 진행되기 때문이다. 또한 종양이 성장할수록 에코가 증가하여 등에코(isoehoic) 또는 고에코가 되는 경향이 있어서 간암을 주변 정상 간조직으로부터 구분하기 힘든 경우도 있다[7]. 간암의 석회화도 드물게 보고되고 있으며[8], 때로는 소간암이 지방 변성(fatty metamorphosis)이나 동확장(sinusoidal dilatation)에 의해서 전체적으로 희미한 고에코로 나타나기도 하는데 이런 경우 국소적 지방 침윤(focal fatty infilteration), 해면상 혈관종(cavernous hemangioma) 그리고 지방종(lipoma)과의 감별이 필요하다(Fig. 9) [5,9,10]. 또한 간세포 암종이 다발성 결절로 고에코와 저에코로 동시에 산재되어 나타나기도 한다(Fig. 10). 때로는 간세포암종이 종괴 형태가 아니라 침윤형으로도 나타나며 간문맥을 침범하는 경우도 있으므로 만성 B형간염 등의 만성 간염이나 간경변을 포함한 만성 간질환 환자에서 간실질의 음영이 저에코를 동반하며 주위 간실질과 불균일한 소견이 관찰될 때는, 반드시 간문맥도 항상 같이 확인하여 침윤형 간세포암종의 간문맥 침범 여부를 확인해야 한다(Fig. 11). 다발성 재생결절을 동반한 간경변 환자에서는 미만형 간세포암종이 동반되면 초음파로 구별이 어렵다. 이때도 마찬가지로 간문맥과 간정맥 등을 항상 같이 확인하여 혈관의 침범 여부를 확인하며, 알파태아단백과 protein induced by vitamin K absence/antagonist-II (PIVKA-II) 등의 종양표지자와 CT를 시행하여 미만형 간세포암종의 유무를 확인해야 한다(Fig. 12). 우측 횡경막 아래 간의 dome 부위에서는 장내 가스에 의한 방해가 있거나 피검자가 비만인 경우 병변을 관찰하기 어렵다. 간세포암종 진단을 위한 복부 초음파의 정확성에 관한 메타분석에서 민감도는 78% (95% CI 60-89%)였고, 특이도는 89% (95%CI 80-94%)였다[11].

간내담관암(intrahepatic cholangiocarcinoma)

담관암은 담도 상피세포에서 생기는 악성 종양으로 그 위치에 따라 간내 담도의 2차 분지 이하에서 발생하는 간내 담관암과 담도 합류 부위 및 근위부 총간담관에서 발생하는 간문부 담도암 그리고 원위부 총간담관 이하에서 발생하는 간외 담도암으로 나뉜다. 간내 담관암은 모든 원발성 간암의 10%와 담관암의 10%를 차지한다. 간내 담관암의 초음파 소견은 내부 에코는 다양하여 비특이적이지만, 대부분 저에코이거나 타겟 사인(등에코의 중심부와 저에코의 주변 테두리)을 보인다(Fig. 13) [12]. 외연은 불규칙하며 종괴가 커지면 주위로 위성병변(satellite lesion)을 동반하기도 한다. 뮤신이나 석회화된 병변을 동반할 경우에는 후방에코 그림자를 동반하는 고에코로 관찰된다. 30% 정도에서 병변 원위부의 간내 담도 확장을 보이는데 이러한 소견은 다른 악성 간종양과의 감별진단에 도움을 준다(Fig. 14). 또 간외 담관의 확장 없이 간내 담관의 미약한 확장 및 비후된 담관벽을 보이는 간흡충증을 시사하는 초음파 소견이 관찰되어도, 간내 담관암의 진단에 도움이 된다.

전이성 간암(metastatic liver cancer)

일반적으로 전이성 간암의 초음파 소견은 다양한 크기의 다발성 종괴로 종괴 주위에 저에코의 달무리(hypoechoic halo)를 갖는데, 이를 Bull’s eye 또는 타겟사인(target sign)이라고 하며, 전이성 간암의 특징적 소견이다(Fig. 15). 이러한 소견은 공격적인 성향의 암종을 시사하나, 전이성 간암에서만의 특징적인 소견은 아니며 일차성 간암인 간세포암종 등에서도 관찰될 수 있다. 그러므로 종괴 주위로 저에코의 달무리가 관찰되면 악성 종괴 여부 확인을 위해서 CT나 magnetic resonance imaging 등의 확진을 위한 추가 검사를 진행해야 한다. 종괴 주위의 저에코 달무리를 조직학적 검사를 통해서 확인해 보면 정상 간실질이 빠르게 성장하는 암 종괴에 의해서 압박을 받는 소견을 보이며, 드물게는 증식하는 암세포의 섬유화나 혈관 생성화를 나타내기도 하며, 섬유화된 변연이기도 하다[13-15]. 전이성 간암을 잘 일으키는 암들은 담낭암, 대장암, 위암, 췌장암, 유방암, 폐암 등으로, 대부분의 전이는 간동맥이나 간문맥을 통한 혈행 전파로 이루어지지만, 위암, 췌장암, 난소암 등에서는 림프계 전이를 통해서 전이되기도 한다. 전이성 간암은 대개는 다발성으로 나타나지만 단일 종괴로 발견되기도 한다. 이러한 전이성 간암은 미만성 간질환이 있거나 혈관종과 같은 다른 종괴가 있는 간에서도 나타날 수 있기 때문에 진단에 주의를 요한다.
전이성 간암은 초음파에서 에코 패턴에 따라서 고에코(echogenic), 저에코(hypoechoic) 그리고 미만침윤형(diffuse infiltrative) 형태로 나타날 수 있다. 초음파상 고에코를 보이는 전이성 간암의 특징은 혈관성이 많이 발달해 있는 암들로 대장암(colorectal cancer), 신장암(renal cell carcinoma), 신경내분비종양(neuroendocrine tumors), 카르시노이드(carcinoid), 융모막암종(choriocarcinoma) 그리고 섬세포암종(islet cell carcinoma) 등에서 나타나기 쉽다(Fig. 16) [16,17].
초음파상 저에코를 보이는 전이성 간암은 일반적으로 저혈관성(hypovascular) 타입으로 유방암, 폐암, 위암, 췌장암, 식도암 등에서 관찰되며 림프종 때도 이러한 소견을 보인다(Fig. 17).
Bull’s eye 또는 타겟 패턴은 전이성 간암에서 흔히 나타나는 비특이적인 소견으로 모든 전이성 간암에서 자주 관찰할 수 있으나 기관지원성암종(bronchogenic carcinoma)에서 특히 더 잘 관찰된다[18]. 미만침윤형 패턴은 비교해야 할 정상 간실질이 거의 없기 때문에 초음파 상으로 감별하기 가장 어려운 경우인데 유방암, 폐암 그리고 악성 흑색종(malignant melanoma)에서 전이될 경우에 가장 흔하게 관찰할 수 있다(Fig. 18).

악성 낭종성 간종괴(malignant cystic liver mass)

간 혈관육종(hepatic angiosarcoma)

혈관 내피세포에서 기원하는 매우 드문 악성 종양으로 60-70대의 남성에서 호발하며 생존기간은 진단 후 3-6개월이다. 대부분이 다발성이며 내부 출혈이 있을 수 있다. 초음파 소견은 고에코의 결절로 보이거나 혼합 에코의 큰 종괴로 보일 수 있으며 혈관종과 비교하여 훨씬 혈관성이 풍부하고 도플러 초음파에서 내부에 혈류의 흐름이 관찰된다(Fig. 19).

담도낭선종/낭선암종(biliary cystadenoma/cystadenocarcinoma)

매우 드문 낭종성 병변으로, 원인은 잘 알려져 있지 않지만 이상 담관에서 생기는 선천성 질환으로 간에 낭성 종괴를 형성하며 내부에 점액성 액체가 차 있다. 60세 전후의 여성에서 호발하며, 담도낭선종(biliary cystadenoma) (Fig. 20)에서 담도낭선암종(biliary cystadenocarcinoma) (Fig. 21)으로의 악성 전환이 흔하므로 수술적 절제가 치료 원칙이다. 단순성 낭종과 비교 시에 격벽(septum)이 있으며, 낭종에 의한 낭종 상부 담관의 경미한 확장이 관찰되거나, 낭종벽이나 격막에 고형성 결절(mural node)이나 석회화가 동반되는 경우, 담도낭선종/낭선암종의 가능성을 시사한다[19,20]. 담도낭선종과 담도낭선암종을 영상의학적 검사만으로 감별하기는 어려우나, 격벽에 결절이 존재 시에 담도낭선암종을 더 고려할 수 있다[21,22].

결 론

간종괴의 복부 초음파 진단은 비침습적이며, 빠르고 간단하게 시행할 수 있는 효과적인 검사로 일차적인 간종괴의 진단에 있어서 매우 유용하다. 종괴 주위의 저에코의 달무리(peripheral hypoechoic halo)와 타겟사인은 간세포암종과 전이성 간암에서 보이는 특징적인 소견으로 악성을 시사하며, 낭성 종괴에서 격벽(septum)을 동반하고, 낭종 상부의 담관 확장, 또는 낭종벽이나 격막에 고형성 결절(mural node)이나 석회화를 동반 시에는 낭성종괴의 악성 병변을 고려해야 한다. 복부 초음파검사만으로 간종괴의 양성과 악성의 감별에는 제한이 있지만 양성과 악성 간종괴의 특징적인 복부 초음파 소견을 이해하면 복부 초음파의 간종괴 진단을 효과적으로 시행할 수 있다.
