초음파의 생물학적 위험성: 진단적 초음파 중심으로

Abstract

Among the two distinct medical ultrasound applications, diagnostic and therapeutic ultrasound, the diagnostic application is widely used for non-invasive imaging to acquire both anatomical and functional information from inside the body. The advantages of diagnostic ultrasound are safety due to absence of ionizing radiation, patient-friendly convenience, and portability compared with other medical imaging tools. Even though diagnostic ultrasound is considered safe, the ultrasound beam could interact with tissue and produce biological effects. Thus, it is important for the ultrasound operator to understand possible ultrasound risks and apply safety precautions while monitoring the thermal or mechanical index if applicable. This article provides basic information about the biological effects of ultrasound and reviews the possible biological hazards.

서 론

진단적 초음파(diagnostic ultrasonography)는 비침습적으로 인체 내부 장기의 구조와 기능을 평가할 수 있는 영상 진단기법으로서, 의사의 청진기처럼 보편적인 검사로 인식되고 있다. 진단적 초음파는 단순 방사선촬영(X-ray) 또는 전산화단층촬영(computed tomography) 검사와는 달리 이온화 방사선이 발생되지 않는다는 안전성과 응급실 또는 중환자실 침상에서 수행하여 바로 정보를 얻을 수 있다는 유용성을 갖고 있어서[1,2], 태아 및 산과 초음파, 심장 초음파, 외상 초음파, 침상 초음파, 세침흡인 유도 초음파 등 다양한 임상 환경에서 이용되고 있다.
여기서 초음파란 인간의 가청 영역(20-20,000 Hz)보다 더 높은 주파수를 갖는 파동을 말한다. 일반적으로 영상 진단에 이용되는 초음파 주파수는 1-30 MHz 범위이다. 초음파가 침투할 수 있는 깊이는 주파수가 증가할수록 감소하기 때문에, 관찰 장기가 탐촉자(transducer)로부터 멀어질수록 낮은 주파수 범위가 적용된다(예를 들면, 성인 경흉부 심장 2-3 MHz, 복부 3-5 MHz, 유방 및 갑상선 7.5-13 MHz 등) [3]. 진단적 초음파기기는 인체에 초음파를 발신한 후, 전파(propagation), 반사(reflection), 굴절(refraction), 산란(scattering), 흡수(absorption) 등을 거쳐 결국 탐촉자로 되돌아오는 반사파를 검출하고 적절한 신호 처리를 통하여 초음파 영상을 구현한다[4]. 이러한 과정에서 인체 조직은 필연적으로 초음파에 노출되며 조직 내 생물학적 영향이 야기된다. 그러나 안타깝게도 초음파의 안전한 사용 기준에 대한 교육은 부족하여, 산부인과 레지던트의 13.4%, 산과 전임의의 20.9%만이 산과 초음파 검사의 안전성에 대한 기준을 알고 있었다는 미국 설문조사도 있다[5]. 진단적 초음파를 다루는 의사는 인체에 탐촉자를 대기 전에 초음파가 조직 내로 침투할 때 어떤 생물학적 영향이 있을 수 있고, 어떠한 환경에서 인체에 위험성이 증가하며, 어떠한 주의가 필요한지를 미리 숙지할 필요가 있다.
이에 저자들은 초음파로 인해 발생하는 생물학적 영향의 주요 지표, 생물학적 효과의 종류, 진단적 초음파 장비의 안전한 사용을 위한 지침에 대해 고찰하고자 한다.

본 론

용어의 정의

초음파의 생물학적 영향을 이해하기 위해 열지수(thermal index, TI)와 기계적 지수(mechanical index, MI)를 이해해야 한다. 임상에서 실제로 운용되는 진단적 초음파에서 볼 수 있는 TI, MI 예를 들었다(Fig. 1).
TI는 초음파에 노출된 조직에서 일어날 수 있는 최대 온도 상승치를 의미한다(TI = W/Wdeg) [6,7]. W는 초음파 탐촉자에서 발생한 음향 파워(acoustic power)이고, Wdeg는 초음파로 조직의 온도를 1°C 상승시키는 데 필요한 힘이다. 즉, TI = 1값은 현재 초음파에서 발생하고 있는 음향 파워가 조직 온도를 최대 1°C 상승시킬 수 있는 정도라는 것을 의미한다. TI는 조직에 따라 세분화하여 표시된다: 연부 조직 열지수(thermal index for soft tissue, TIS), 골 열지수(thermal index for bone, TIB), 두개골 열지수(thermal index for cranial bone, TIC) [7].
MI는 초음파에 대한 노출이 비열적 기전(nonthermal mechanism)에 의해 생물학적으로 불리한 영향을 미칠 가능성을 정량화한 것으로서, 생체 내 추정된 최대 희박 압력을 빔 중심 주파수의 제곱근으로 나눈 값으로 정의된다(MI=Peak rarefactional pressureOperating frequency) [6]. MI값이 클수록 조직내 관성 공동화(inertial cavitation)에 의한 효과가 크다고 예측할 수 있다.

초음파의 생물학적 효과(biological effects)

인체 조직에 초음파가 노출될 경우 발생하는 생물학적 영향은 크게 열효과(thermal effect)와 비열효과(non-thermal effect)로 나눌 수 있다(Table 1).

열효과(thermal effect)

초음파의 열효과로 조직 가열(tissue heating)의 위험성이 있다. 초음파는 인체 조직을 투과 시 조직의 점탄성 흡수(viscoelastic absorption) 작용으로 인해 초음파 파동의 진폭이 감소한다. 줄어든 진폭의 에너지는 조직 내에 열에너지 상태로 흡수되어 조직의 온도를 상승시킨다. 조직의 열 증가값은 2αI/C의 공식으로 정의된다(α: 조직의 진폭 흡수지수, I: 초음파의 강도, C: 매질의 비열) [6]. 조직의 성분 중 지방과 콜라겐의 양은 α값을 증가시키며 물의 양은 α값을 감소시킨다. 그래서 α값은 물에서 가장 낮고 골조직에서 가장 높다(물 < 체액 < 연부 조직 < 피부, 연골 < 태아골 < 성인골) [8].
골조직은 초음파의 파동을 많이 흡수하여 많은 열에너지를 발생시키게 되는데, 이렇게 발생한 열에너지는 골 주변과 내부 골수의 온도를 상승시킨다. 특히 골조직과 연부 조직의 경계 면에서 많은 열에너지가 발생되는데, 임신 10주차 태아의 골화(ossification) 발생 이후 간헐 파형 도플러(pulsed-wave Doppler)와 같이 고에너지가 발생되는 초음파 적용 시 온도 상승에 주의해야 한다. 열은 동물에서는 잘 알려진 기형 유발 물질(teratogen)이며, 태아의 신경 발생 시기 모체 온도 2°C 상승은 소뇌증, 소안구증, 뇌 발달 지연 등을 야기함도 보고되었다[9]. 열 피해의 심각성은 세포 수준에서 단백질 합성 저하, 세포 회전율(cell turnover) 약화, 세포 사멸(apoptosis) 증가로 나타날 수 있다[6].
진단적 초음파에서 열효과 정도는 TI값으로 평가한다. 일반적으로 TI ≤ 0.7은 초음파 노출시간과 무관하게 인체에 안전하지만, TI ≥ 6에서는 1분 이상 초음파 노출 시 인체 조직에 손상을 야기한다[1]. 진단적 초음파 사용 시, 조직 가열을 최소화하려는 노력이 필요한데, 산과 초음파 검사와 경두개 도플러 검사 시 TI와 초음파 노출시간을 주의 깊게 관찰할 필요가 있다.

비열효과(non-thermal effect)

비열효과는 기계적 효과(mechanical effect)라고도 하는데, 대표적인 예로 공동화 효과(cavitation effect)를 들 수 있다. 초음파 검사 시 인체 내로 음압(acoustic pressure)이 발생한다. 액체 내의 기포는 이러한 음파의 압력 변화를 접하면서 크기 변화를 겪게 되는데, 압력이 감소하는 동안 팽창하고 음파의 압축 반주기(compression half-cycle) 동안 수축하는 형태 변화, 즉, 음향 공동화(acoustic cavitation) 현상을 만들게 된다[10]. 초음파 영상에서 공동화 현상은 기계 판막 주변에서도 관찰할 수 있는데(Fig. 2), 기계 판엽을 통과하는 혈액의 흐름(squeeze flow)으로부터 야기되며, 판막의 닫힘 속도가 빠를수록 공동화가 잘 일어난다[11]. 최고 음파의 압력이 증가함에 따라 상이한 변화가 유도될 수 있는데, 결국 기포는 수축하면서 불안정해지며 주변 액체의 관성으로 붕괴될 수도 있다. 이러한 불안정한 기포 반응을 특별히 관성 공동화(inertial cavitation)라고 일컬으며, 일반적인 안정 또는 비관성 공동화와 구별된다.
관성 공동화 발생 시, 열과 기계적인 힘으로 인해 유리 산화기(free oxygen radical) 합성과 전단력(shear force) 증강이 흔히 동반된다. 기포가 터지는 순간 기포 주변 온도는 5,000 Kelvin을 넘으면서 유리 산화기를 형성하게 되는데, 유리 산화기는 세포 내 DNA를 손상시켜 인체 내 노화, 암 등을 발생시킬 수 있다[1]. 기포 파열은 주변 조직에 전단력를 전달하여 모세혈관의 파열을 유발할 수도 있는데, 혈우병 또는 항혈소판제 복용 중인 환자에서는 모세혈관 파열로 인한 혈액의 혈관 외 누출이 문제가 될 수 있다[12]. 조영 심초음파 스캔 동안 심실기외수축이 유도되었다는 보고도 있다[13].
진단적 초음파에서 관성 공동화 효과의 정도는 MI값으로 예측한다. 예를 들어, 관성 공동화로 인한 위험은 조영제(gasbubble contrast agents) 사용과 관련이 있는데, 특히 조영제가 파괴되는 소위 높은 MI > 0.7 스캔 환경에서 증가되므로, 숙련된 초음파 검사자는 MI값을 보고 스캔 방식을 조정할 수 있어야 한다. 일반적으로, 조영제 초음파 검사 시 MI < 0.4로 진행하여 관성 공동화 현상이 발생하는 것을 최소화하려는 노력이 필요하다[14].

진단적 초음파 장비의 안전한 사용을 위한 지침

진단적 초음파 사용 시 as-low-as-reasonably-achievable (ALARA) 원칙을 지켜야 한다[15]. ALARA 원칙이란 합리적으로 달성할 수 있는 가장 낮은 수준으로 유지해야 한다는 것이다. 이는 방어적인 원칙으로서 가능한 가장 낮은 초음파 에너지를 사용하여, 가능한 가장 짧은 시간의 초음파 노출을 해야 한다는 권고이다. 실제적으로 초음파 검사 시 음향 파워가 가장 낮은 값으로 검사를 시작하여 점차 음향 파워를 높여가면서 영상 검사를 진행해야 하며, 초음파 탐촉자는 환자의 동일한 부위에 가능한 짧은 시간 접촉해야 한다. 임상에서 특히 ALARA 원칙을 신중하게 고려해야 할 대상 세 가지는 눈, 폐, 태아이다[1]. 눈은 열효과에 취약한 장기로서 TI > 1.0에서의 초음파 노출은 금기이고, 폐 표면은 마른 체형 등에서 비열효과로 인한 모세혈관 출혈 위험성이 있으며, 태아의 열효과 위험은 주지의 사실이다[1].
이에 영국 의학초음파학회(British Medical Ultrasound Society, BMUS)는 초음파 장비의 안전한 사용을 위한 핵심 원칙들을 제시하고 있다[16]. 즉, 의료 초음파 영상은 의료 진단을 위해서만 사용해야 하며, 임신 중 스캔은 기념을 위한 비디오 사진 제작의 목적으로 수행되어서는 안된다. 초음파 장비는 안전하고 적절한 작동법 교육을 받은 자가 사용해야 하는데, 사용자는 초음파의 잠재적인 생물학적 효과(열효과, 비열효과)와 기계 세팅이 인체에 미치는 영향을 알아야 한다. 출력(output) 수준과 스캔시간은 유용한 영상 진단을 달성할 수 있는 한 낮고 짧게 유지되어야 한다. 검사자는 BMUS 권장 스캔시간을 준수해야 한다.
초음파의 열효과 정도의 평가 지표인 TI값에 대한 권고는 대상에 따라 차이가 있다. 배아 또는 태아는 열효과에 취약하기 때문에, 산과 초음파 검사 시는 TI를 모니터링하여(마지막 생리 10주까지는 TIS, 이후에는 TIB) 이에 따른 허용 가능한 스캔시간을 준수해야 한다. 예를 들면, ALARA 원칙하에 TI ≤ 0.7에서는 스캔시간을 제한하지 않지만, 2.5 < TI ≤ 3.0에서는 1분 미만으로 제한하고, TI > 3.0에서는 스캔을 권하지 않는다(Table 2). 신생아에서는 경두개 및 척추 초음파 검사 시, TIC > 3.0이 되지 않도록 해야 하고, 기타 신생아 일반 초음파 스캔 시에는 TIB > 6.0이 되지 않도록 주의해야 한다(Table 2). 성인에서는 경두개 초음파 검사 시, TIC > 3.0이 되지 않도록 해야 하고, 기타 일반 복부 초음파 등에서도 TI값(영상에서 뼈가 1 cm 이내 존재하면 TIC, 뼈가 전혀 나오지 않으면 TIS, 그 외는 TIB)이 6을 초과하지 않도록 권고하고 있다(Table 3). 초음파의 관성 공동화 정도의 평가 지표인 MI값에 대한 권고는 검사 대상과 초음파 종류에 따라 큰 차이는 없다. MI ≤ 0.3에선 스캔시간에 제한이 없고, MI값 0.7을 초과할 때 조영제 사용 시 관성 공동화 현상을 경고하고 있는데, 이론적으로는 조영제를 사용하지 않는 초음파 검사에서도 높은 MI값에서 공동화 발생 위험이 있다(Tables 2, 3).
보다 구체적으로 임상 초음파 적용에 따라, 2020년 BMUS는 진단용 초음파 장비의 안전 사용 지침을 산과 초음파(Table 2)와 비산과 초음파(Table 3)로 나누어 제시하였고, 이 중에서 TI, MI, 노출시간 제한이 있는 경우는 반드시 숙지할 필요가 있겠다[16].

결 론

현재 임상에서 널리 사용되고 있는 산과 초음파를 포함한 진단적 초음파의 생물학적 위험도는 낮다. 그러나 초음파의 생물학적 영향(조직 가열, 관성 공동화)은 존재하며, 정도는 전자는 TI, 후자는 MI로 평가할 수 있다. 산과 초음파 검사와 경두개 도플러 검사 시 TI 및 노출시간, 조영 초음파 검사 시 MI 및 노출시간에 주의를 기울일 필요가 있다. 특히 눈, 폐, 태아 초음파에서 ALARA 원칙의 준수가 요구된다. 결론적으로 초음파는 안전하면서도 효과적인 진단 영상 기법이지만, 생물학적 영향을 고려한 사용 지침을 숙지하고 지표들을 모니터링하면서 목적에 맞게 운용해야 하겠다.
